3. Fine Speed Control Matters More Than You Would Think
An underrated specification: the increment by which the machine changes speed. Most treadmills adjust in 0.1 mph steps, which is fine. Some budget machines move in 0.5 mph steps, and for this use that is a genuine problem.
The reason is that the comfortable range for an irritable knee is often narrow. It shares the 1750's deck, which measured among the best for recovery. It also runs quieter under sustained load, because the larger motor is working further inside its envelope. The question is whether that is worth several hundred dollars over the 1750, and the answer depends entirely on whether steep sustained walking will be your main session. The handrails run the full length of the deck at a height that supports rather than stoops, and — the detail we rate most highly — speed and incline controls sit on the rails themselves, so you never have to let go and reach forward to slow down. 6. Should You Run at All?
This is a question for your clinician rather than for us, but the equipment side of it is worth setting out, because the answer changes which machine to buy.
If running is off the table for now, deck length stops mattering almost entirely and the money is better spent on cushioning quality, incline range and speed control. A 55-inch deck is ample for walking, and that unlocks cheaper machines and easier folding.
If you are working back toward running, buy the 60-inch deck now. Returning to running on a deck that is too short forces a shortened stride at exactly the point when your gait needs to be as normal as possible, and that is a poor combination for a knee you are trying to protect. 7. When Another Machine Is the Better Answer
The honest version of this guide has to say when a treadmill is the wrong purchase, and for some readers it will be.
If walking on a flat surface for twenty minutes is painful, adding a moving belt does not change the fundamental problem — you are still weight-bearing, and the loading is what hurts. A stationary or recumbent bike removes the impact entirely while allowing considerably more work to be done, and for genuinely irritable knees that is usually the better starting point. Two variables affect load through the knee as much as the deck does, and both are cheaper to change than the machine.
Footwear is the first. Worn-out shoes have lost most of their midsole compression, and a shoe with 600 miles on it is a materially harder surface than the same shoe new. Replacing shoes is the least expensive intervention available and frequently the most effective. Whether a particular shoe type suits you is a question for a physiotherapist rather than a treadmill review.
The second is what the treadmill stands on. A machine on thick carpet shifts under load, and a frame that moves slightly on each footfall introduces small instabilities your knee has to absorb. 9. A Cautious First Six Weeks
Equipment aside, the pattern that goes wrong is consistent: an enthusiastic first week, a flare-up, a fortnight off, and a conclusion that treadmills do not suit you. A slow build avoids it and costs nothing but patience.
Weeks one and two: ten to fifteen minutes of flat walking at a speed that feels almost too easy, three times a week. The goal is to establish that the activity is tolerated at all. Weeks three and four: twenty to twenty-five minutes, and introduce 2 to 3% incline in one session only.Weeks five and six: thirty minutes, with incline in two sessions, building a percent at a time.
